You are considered a new or first time applicant if you answer yes to any of the following:

1. You are applying for a US passport for the first time.
2. Your expired US passport is not in your possession.
3. Your previous US passport has expired and was issued more than 15 years ago.
4. Your previous US passport was issued when you were under 16 and you are now over 16

1. I can submit my most recent passport.
2. I was at least 16 years old when my most recent passport was issued.
3. I was issued my most recent passport less than 15 years ago.
4. I use the same name as on my most recent passport, OR, I have had my name changed by marriage or court order and can submit proper documentation to reflect my name changes.
5. My current passport is undamaged.

This form should only be used for making a data change for a previously issued, currently valid U.S. passport WITHIN one year from the date of issue:

1. When there is a change in your identifying information, such as a name-change due to marriage or by court order;
2. To extend the validity of certain previously limited passports; or
3. To correct a Passport Services’ printing error.

If it is MORE than one year since the date of issue , you must submit Form DS-82 (Renewal) (or DS-11, if appropriate)
